Description

As many clinicians who treat Scoliosis are aware, the SRS-22 and SRS-30 are key QOL questionnaires used in the clinic. Processing large volumes of printed paper and hand written answers can be challenging and prone to errors when entering data into a larger database.

This app aids in simplifying this process, where a staff can pass the iPad to the receipient to answer the questionnaire, then collect the device, securely compute the domain scores requiring authentication, and outputs the result as a QR Code. The QR Code is in a standard UTF-8 CSV format, so it can be copied on to a text file, rename to a CSV file and is in a readily readable in most spreadsheet programs.

No personal identifying information will be saved per session once the QR Code window is closed, and therefore comply with many institution's requirements for data protection. (Please refer to individual institution's policy before use of this app).

Note: This app is not designed to be used on an iPhone. Please use an iPad instead.
