This app integrates with the HealthKit app to retrieve activity data, and uses this data to track patient activity levels and recovery.

Not fully functional
Unable to complete and send information to doctor. Plus I’m not seeing the complete wording for each question.
Doesn’t work
There is no send button and the UI is flakey.
